This is an example form on which to base future forms.
First, insert a "Confiforms Form Definition" like so and select "Other macros"
Then you can search for the Form Definition on the top right. Fill out the fields as you deem necessary.
Next, you will want to define Field Definitions. Inside the Confiforms Form Definition add one per field type you want in your form, again by opening the macro browser and searching for "Field Definition".
Then add a "Confiforms (FormView) Registrations Control". This is where you will place your actual fields. When filling out the Registrations Control macro make sure to select "embedded" as the rendering mode and enter the same FormName as in the confiForms Form definition.
Finally add as many Confiforms fields as you wish, making sure to use the same fieldName as in the field definitions you added above.
Please note the Preview will not render the form correctly. Just press Save to view the form.
